Patient Health Information

How Patient Health Information Is Used
Personal health information is used for rendering treatment, obtaining authorization and payment for treatment rendered, administrative purposes, and evaluating the quality of care received. In certain circumstances, disclosure of this information is appropriate even without patient consent.
Instances where patient consent is not required may include, but are not limited to:

- Public health activities
- Serious threats to the health or safety of the public or individuals
- Workers’ compensation cases
Your written authorization will be requested as a standard prior to any release of your patient health information. If you choose to sign authorization to disclose information, this authorization can be revoked at any time.
